Care Planning, Referral and Admission

Young People age 16-24 years may be referred to Optimum Independent Services via telephone, email or via our website. A Risk Assessment from the referring agency is required, this will be followed by an internal risk assessment by an in-house Qualified Social Worker. Referrals are accepted from Local Authorities and also directly from the Home Office.

Upon arrival, an inventory of the young person’s possessions is prepared. If the young person arrives with inadequate or damaged clothing and agreed budget will be agreed to purchase essential items such as additional clothing, toiletries and other personal items at the earliest opportunity.

A Placement Agreement Meeting is to be scheduled and convened within five working days, but where possible held on the same day of admission.

Care Plans are reviewed and revised as required and is achieved in collaboration with the young person and their key professionals from both private and statutory agencies. Huge emphasis is placed upon the Young People participating in the development of their own Care Plans to promote a sense of ownership and accountability toward their own goals and targets.
